Community Hospice & Palliative Care

Established in 1979, Community Hospice of Northeast Florida is the only locally based, nonprofit hospice provider serving residents of Baker, Clay, Duval, Nassau and St. Johns counties. Each day, more than 850 employees and 900 volunteers help approximately 1,300 patients and their families live better with advanced illness. Community Hospice has served nearly 82,000 patients at home, long term care and assisted living facilities, in hospitals and its six inpatient care centers conveniently located throughout the area.
